Responsibilities:
1. HHA is responsible for providing patients with in-home personal care and designated health-related services
2. Provides care according to the aide plan of care|assignment sheet and in accordance to State Practice Act or regulations
3. Reports any unusual circumstance in the patient's condition or home environment to the primary Care Manager
4. Follows currently written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs)
5. As assigned, performs incidental household services essential to the patient's health care at home to prevent or postpone institutionalizatio
6. Records and notifies office Care Manager of any variations from the established parameters or changes in the patient's condition
Qualifications:

1. Conduct comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors, to develop individualized care plans.
2. Perform skilled nursing procedures and treatments as prescribed by the physician or advanced practice provider, such as wound care, medication administration, injections, catheter care, and monitoring vital signs.
3. Monitor patient's health status, symptoms, and response to treatment, and communicate any changes or concerns to the supervising registered nurse or healthcare provider.
4. Educate patients and caregivers on disease management, medication adherence, self-care techniques, and preventive measures to promote optimal health and independence.
5. Provide emotional support, counseling, and reassurance to patients and families coping with illness, disability, or end-of-life issues, demonstrating empathy, compassion, and cultural sensitivity.
