Who Do We Care For?

Fountain of Hope Homecare Services provides home and community-based services for adults and
children diagnosed with intellectual disability and autism. We are qualified to provide services through
the Office of Developmental Programs in Pennsylvania.

Fountain of Hope Home Care Services exemplifies compassion, dependability and dignity to all of our clients. Our company was founded on the principles of caring for others in a way that we would also want to be cared for. We provide medical and non-medical in-home healthcare in the privacy of a client’s home.

Pride in Professional Service

Our services are provided to anyone, in any situation that makes living independently more difficult. Aging, illness, or disabilities can put a person in an unexpected state of home care needs which often times can create an urgency for care. We are prepared and ready to step in to offer the care you or your loved ones deserve. Our trained and experienced caregivers have the compassion and skills necessary to help overcome the challenges you and your loved one may be facing.

Community Integration

We provide opportunities and support for community inclusion and developing skills. We offer our clients opportunities and support to participate in community activities according to their preferences, choices, and interests.

Companion Service

We provide companion service to individuals who need supervision or assistance to maintain their health, safety, and welfare. This service can be provided in our clients' home or community setting including workplace.

Respite Services

We provide respite services and support on a short-term basis for planned or emergency situations. We are qualified to provide respite services in our clients' private homes or outside of their home.

In-Home and Community Services

We provide in home and community support to assist our clients in acquiring, maintaining, and improving the skills necessary to live in the community, gain more independence and participate meaningfully in community life.

Nursing Services

We provide medically necessary nursing service in accordance with the PA State Board of Nursing. Our services include health teaching, health counseling, provision of care supportive to or restorative of life and well-being and executing medical regimens as prescribed by a licensed physician or dentist.

Homemaker Chore

Our homemaker service enables our clients, family member or friend with whom our clients reside with to maintain their primary private home. These services include cleaning and laundry, meal preparation general household care, infection control measures, cleaning of medical equipment and disinfecting the home.

Alzheimer’s, Dementia and Cognitive Change Services

As a family member or friend of someone with Alzheimer’s disease, dementia or cognitive change, you shoulder a particularly heavy burden.

Specialized Home Care

We offer specialized home care services through our Life Care Navigation program. If your loved one needs distinct care, we will combine these specialized services with our traditional home care to develop a personalized plan.

Non Medical Home Care

Personal independence is important at any age, but especially for seniors who sometimes feel like they’ve lost other freedoms like driving, traveling alone or entertaining family and friends.

Health Reminders

Our Fountain of Hope home healthcare caregivers understand the importance of protecting your loved one’s health and safety in the little things of everyday living.

Transportation

Our non-emergency transportation services enable our clients to access services and activities in the community.
